Drug-interaction alerts overridden by default

System failure identification
Interaction warnings are overridden at very high rates, frequently without the reason field completed. The safety net is present in the record and absent in practice. The override becomes reflexive precisely because it is required so often.

Data pipeline diagnostics
Override reasons are captured as free text or an optional field, so the data that would separate a considered override from a reflexive one is unusable in aggregate. Unstructured reasons cannot be counted, so the override pattern is never analysed.

Predictive computational model
Make the reason a structured, required selection for a defined subset of high-severity interactions only, then analyse the distribution. Narrow the requirement so it does not become the next thing dismissed. A narrow scope keeps the friction proportionate to the severity of the interaction.

Workflow integration
Apply the requirement to the smallest rule set that justifies the friction. Expanding it universally would reproduce the fatigue the intervention is meant to reduce. Universal friction is how the current problem was created in the first place.
